Will be responsible for regularly scheduled duties in the care and treatment of residents following established procedures.

Review Face sheet and all discharge paperwork, contact physicians as necessary to clarify any unclear findings

Transcribe resident's orders including a diagnosis for all meds and fax to pharmacy & doctor's office (include on orders: weekly weights on weekly shower shift, wound care if any, follow up appointments if any and treatments)

Complete infection tool form when necessary

Take report on new admission, including reviewing any discrepancies found during review of discharge paperwork

Communicate with floor staff of new admission (arrival time, room and dx) Interview and collect all necessary information from resident on all admissions

Able to orient the resident to the facility, room and routine

Provide residents with education regarding their dx, wounds, overall health etc

Able to admit admissions into SigmaCare

Communicate with physicians regarding new admissions

Complete the admissions tool form including date and initial of all tasks completed

Able to enter all CNA tasks in SigmaCare Accurately complete Braden Scale, Oral Assessment, GG form, Hepatitis Screening and Dental services form

Complete thorough head to toe Skin Assessment with accurate measurements and enter into SigmaCare

Proficient in checking, adding and reviewing orders in QuickMarAble to clearly transcribing any new orders necessary after completing skin assessment

Complete and deliver diet slips and swallow evaluation if necessary

Able to accurately complete CNA care sheet

Complete RN assessment note in SigmaCare on all admissions

Able to complete medication reconciliation with admission and document completion of this

Enter all care plans in SigmaCare (Pain, Risk to Fall, Potential for bruises/skin tears, incontinence if applicable, vision and dental, psych meds if on any, infection/isolation if appropriate etc)

Able to transcribe Baseline Care plan per policy and review with resident

Complete discharge teaching form

Utilize brief sizing tool and document the correct size brief/pull up on CNA care sheet

Take and record vital signs when necessary

Uses isolation precautions as designated.

Observe and report significant changes in resident's condition during admission process

Communicate with all social workers as necessary

Respect all resident's dignity when performing all tasks

Assist LPN's and CNA's when needed with all tasks

Report conditions requiring housekeeping and maintenance attention

(ie. Bed malfunctioning, resident request to hang items on wall, toilet safety frames, fall mats)

Ensure the safety of residents during assigned shift by following established procedures for medical emergencies and fire as required.

Perform other related duties as assigned.

Qualifications:

Must possess current Registered Nurse license and experience in a long-term care setting is desirable.

Pay: $75,000.00 - $80,000.00 per year
