As a Community Health Worker, you will play a pivotal role in promoting and nurturing relationships both within the community and with healthcare professionals. This multifaceted position involves a combination of community outreach, professional networking, patient education, and administrative responsibilities.

Position : Community Health Worker/ Community Engagement Coordinator

Location : Carnegie, PA- IN PERSON OPPORTUNITY; NOT REMOTE

Hours : Monday to Friday 8 AM to 4 PM or 9 AM to 5 PM

***Will be occasions you will need to be available for evenings, rare weekends - this would be for events or vaccine clinics

Responsibilities:

Community Engagements and Professional Engagements:

Actively engage with residents and stakeholders in various community settings to foster relationships promote our services and cultivate connections with healthcare professionals to enhance collaboration and referral processes.

Home Visits: Conduct educational visits to the homes of new patients to provide information about our program and support their transition into our services.

Referral Management: Facilitate referrals to community-based organizations to ensure patients have access to additional support services as needed.

Vaccination Program Management: Oversee vaccination programs, including handling required paperwork, ensuring accurate data entry, and coordinating logistics to support successful vaccination initiatives.

Special Projects: Collaborate on special projects as assigned by management to contribute to the overall goals and objectives of the organization.

Local Travel: Utilize the provided pharmacy car for daily travel within a one-hour radius to fulfill job duties, including community engagements, professional visits, and other support sites.

Skills & Qualifications:
  • Cultural Sensitivity: Ability to interact with a diverse population in a culturally sensitive manner, respecting individual backgrounds and beliefs
  • Problem-Solving Skills: Demonstrate strong problem-solving abilities to address challenges and find effective solutions in various situations .
  • Customer Service Skills: Provide exceptional customer service to both community members and healthcare professionals, fostering positive relationships and enhancing satisfaction.
  • Communication Skills: Exhibit strong verbal and written communication skills to effectively convey information and engage with stakeholders .
  • Autonomy and Innovation: There is an opportunity to shape and personalize the role, contributing to the growth and development of the position , is that something you are comfortable with?
  • Experience giving injections that would allow you to assist with the vaccine clinics would be preferred but is not required
