Aspirus At Home in Rhinelander, WI is seeking a Home Health Patient Care Coordinator to join our field team!

The PCC provides clinical and administrative supervision to the Home Health location in addition to functioning as a Home Health Registered Nurse. The Home Health PCC is responsible and accountable for the delivery of safe and high quality home health services and is an advocate for the patients under their care.

This position does qualify for the Sign On Bonus.

Qualifications:

• Knowledge of patient care standards and practices acquired through completion of Bachelor's Degree in Nursing, or BSN.
• Active Wisconsin RN nursing license.
• Current BLS certification or obtained within 90 days of hire.
• Valid driver's license required.
• Possesses automobile liability insurance with minimum $100,000/$300,000 coverage required, or $300,000 combined single-limit liability.
• One or more years' experience in home health care preferred

HOURS: Full Time or 1.0 FTE, 80 hours per pay period, varied shifts, on call rotation every 5th weekend.
