Job Description

Purpose of Position:

Provides person-centered, strength-based assessments, evaluating barriers to access and adherence that are tailored to develop treatment/care plans focused on the individual needs and goals of the patient. The assessments will integrate health care and social support services

Responsibilities:

• Develop an interdisciplinary, patient-centered care plan
• Ensure patient with multiple chronic conditions/ disabilities achieve the highest personal level of functioning
• Improve communication among providers/ care team, patients and their caregivers
• Offer a range of services for high risk patients, including psychotherapy and social support
• Support family care givers to reduce stress and burden

Minimum Qualifications

For Assignment to Level II:
1. Valid license as an LMSW issued by the NYSED; or
2. Valid license as an LCSW issued by the NYSED.

Salary may differ based on the licensure (LMSW or LCSW) of the candidate.
