Job Summary:

The Staff Pharmacist III provides professional skills in the distribution of drugs and in clinical services offered to patients and staff. The Staff Pharmacist II performs patient and drug therapy assessment and makes recommendations as appropriate for the care of our patients. They also serve as a preceptor to students.

Major Responsibilities / Duties / Critical Tasks:
• Interprets physician's orders for medications and ensures therapy is appropriate for patient's diagnosis, weight, and age (pediatric, adolescent, adult, and geriatric).
• Assesses for contraindications, drug-drug and drug-food interactions, and allergies, appropriateness of drug and dose and therapeutic duplications.
• Compounds and dispenses prescribed medications per physician's orders in accordance with departmental and applicable laws.
• Maintains accurate and complete patient medication profiles.
• Participates in meeting department financial goals through formulary management, rigid inventory control processes and efficient use of labor and resources.
• Supervises and assists in the activities of supportive personnel.
• Demonstrates and promotes excellence in customer service through teamwork, leadership, decision-making, problem solving and effective communication.
• Engages in MTM activities to meet departmental MTM goals.
• Participates in the quality improvement and medication use review activities of the department.
• Assists departmental manager and director with projects and departmental initiatives.
• Follows institutional 340b policies and procedures.
• Identify and appropriately communicate potential areas of risk or quality issues.
• Participates in the education and training programs to the pharmacy, medical, and nursing staffs and development of new programs to decrease the cost of care. Provide drug and clinical information to health professionals in response to specific questions or relating to new, evolving issues related to the care of patients.
• Maintains records and documents all essential information as necessary.
• Performs related duties as required.

Qualifications

Required Education / Experience:
• Graduate Of an ACPE accredited School of Pharmacy.
• Currently licensed to practice pharmacy in the state of Texas.
