Position Overview: We are seeking a highly organized and detail-oriented Administrative Assistant to join our team. The ideal candidate will play a crucial role in supporting the efficient operation of our Intensive In-Community services by liaising with case managers, handling referrals, and ensuring smooth staffing processes.

Key Responsibilities:
  1. Communication with Case Managers:
  • Maintain regular and effective communication with case managers to understand client needs and address any administrative concerns.
  • Provide timely responses to inquiries, coordinate information, and facilitate collaboration between case managers and our team.
  1. Referral Management:
  • Process and manage the referral intake system, ensuring accurate and complete information is obtained.
  • Coordinate with internal teams to schedule and assign appropriate staff for assessments and service delivery.
  1. Staffing Support:
  • Assist in the coordination of staffing schedules, ensuring proper coverage for Intensive In-Community services.
  • Communicate with field staff to address scheduling conflicts, changes, and updates promptly.
  1. Documentation and Record Keeping:
  • Maintain accurate and organized records of all communication, referrals, and staffing schedules.
  • Ensure compliance with Perform Care and agency documentation standards.
  1. Meeting Coordination:
  • Schedule and organize meetings between therapists and clients
  • Prepare meeting agendas, take minutes, and distribute relevant information as needed.
  1. Administrative Support:
  • Provide general administrative support to the Intensive In-Community services team, including data entry, filing, and other office tasks.
  • Collaborate with other administrative staff to streamline processes and improve overall efficiency.

Qualifications:
  • Strong organizational and multitasking skills with attention to detail.
  • Excellent communication skills, both written and verbal.
  • Proficient in Microsoft Office Suite and other relevant software.
  • Familiarity with Perform Care procedures and documentation standards is a plus.
  • Bilingual- English & Spanish a Plus
  • Evening hours until 7pm required.

Education:
  • High school diploma required; associate's or bachelor's degree in a related field preferred.
