Department Description:

CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.

Job Purpose:

The PACE Administrative Coordinator II performs a wide range of administrative and support functions for the PACE department, including but not limited to serving as a member of the PACE interdisciplinary team; coordinating and scheduling participants with primary providers and specialty care; coordinating transportation for medical appointments; and ensuring documentation in EMR of external provision of care.

Responsibilities:
  • Receives and prioritizes information received from various sources and routes, including but not limited to: participants and their caregivers, PACE team members, PACE Managers and Directors, providers, telephone, EPIC message, email, and contracted partners.
  • Greets departmental visitors in person and via telephone to ensure accurate and timely communication, presenting the department in a positive manner to participants and care partners.
  • Schedules initial and biannual mandatory provider clinical assessments per PACE care plan regulations.
  • Coordinates with providers for schedule and template management.
  • Recognizes and troubleshoots patient-related problems and passes on to appropriate personnel.
  • Documents in EMR per standard related to scheduling and coordination of care including documentation of care provided from outside providers.
  • Enters authorizations for care provided in TPA system per protocols.


Qualifications:
  • High school/GED required; Associates or Bachelor’s Degree preferred
  • Minimum 3-year administrative experience in a medical office setting.
  • Fluency in applications such as advanced word processing, spreadsheets, databases, flowcharts and graphics (Microsoft Office and Google apps).
  • Experience with EPIC scheduling functionality, InBasket, Workqueue.
  • Writing, interpersonal, organizational and communication skills required.
