Over half of the facility is dedicated to skilled nursing/convalescent care while the remaining beds are committed to meet the medical and rehabilitative needs of the short-term patient after a stay at the acute care hospital. Responsibilities

Job Summary:

The Admissions Coordinator performs activities and duties related to pre-admission evaluation and admission of patients to Bruceville Terrace. The goal of the coordinator in this role is to identify patients who meet criteria and who are appropriate for admission to Bruceville Terrace. Included in this role is the ability to review medical records to gather information to complete the pre-admission screening tool to review the appropriateness of the patient for admission with MD collaboration and case management to facilitate transfer in a timely manner; ensures that benefit authorization are completed as required; determines with nursing/others bed and team assignments; builds relationships with external referral sources; gathers/tracks data to be utilized in planning; ensures compliance with regulatory/accreditation organization.

Qualifications

Minimum Requirements:
  • HS/GED required
  • Two years' experience in admissions is required
  • Must possess an in-depth knowledge of admissions processes
  • Bachelor's Degree is preferred
  • Previous experience in an Admissions Coordinator role is preferred
