About Us:
Parkview Care and Rehabilitation Center is a respected healthcare facility dedicated to providing exceptional patient care and a supportive environment for both staff and residents. Situated in South Eastern Massapequa, NY, we are seeking an experienced and proactive ADNS/Risk Manager to ensure the highest standards of safety and compliance within our facility.

Key Responsibilities:
  • Lead comprehensive investigations and reporting of incidents within the facility in strict adherence to New York State Department of Health (DOH) regulations and internal policies.
  • Collaborate with interdisciplinary teams to continuously refine and enhance the effectiveness of the risk management program, providing guidance and education to staff when needed.
  • Maintain and update Occurrence Logs, ensuring all incidents are documented in line with NYS DOH Incident Reporting guidelines.
  • Assess and manage liability risks related to falls, medication errors, abuse, neglect, and other hazardous conditions.
  • Prepare and present detailed reports to the Administrator, Director of Nursing Services, and the Interdisciplinary Team (IDT) regarding ongoing investigations.
  • Develop and implement risk mitigation strategies, evaluating their success and making adjustments to further improve patient safety.
  • Lead safety meetings to foster a culture of safety and accountability throughout the facility.
  • Coordinate investigations and notifications to relevant authorities, ensuring timely and accurate reporting.
  • Represent the facility during audits, inspections, and investigations conducted by governing agencies.

Qualifications:
  • Bachelor's degree in Risk Management, Healthcare Administration, Nursing, or a related field.
  • Prior experience in healthcare risk management is preferred.
  • Strong working knowledge of New York State DOH regulations and Incident Reporting procedures.
  • Excellent analytical, problem-solving, and communication skills.
  • Detail-oriented with the ability to work collaboratively across interdisciplinary teams.
  • Knowledge of healthcare liability and risk management best practices.
  • CPHRM certification is a plus.
