Responsibilities:

The advanced practice provider (Nurse Practitioner/Physician Assistant) within the ambulatory setting works in collaboration with, and is an integral member of, a multidisciplinary health care team. The APP provides comprehensive care in the Cardiology ambulatory practice.
  • Assumes active role in overall HF program development at JNE. Works collaboratively as a team member with HF Specialists / APPs / RNs and ancillary AHPs to oversee population management of Heart failure ( HF) patients via real time identification of inpatients with diagnosis of HF on daily basis, tracks appropriateness of care setting and resource utilization, assists with identification of advanced HF patients, collaborates with clinical staff to ensure optimal GDMT, ensures appropriate discharge plan and coordinates both patient / family and staff education regarding HF. Works with hospital based Quality Improvement staff to maintain metrics on HF outcomes such as readmissions, Length of Stay and HF Quality indicators. Coordinates Quality Improvement processes for HF care. Oversees / provides Transitional care after discharge for patients transitioned to rehabilitation facilities, skilled facilities, personal care facilities, out-patient programs, and home with home care or home as per the Hospital to Home (H2H) guidelines. Provides coaching to clients to enable them to gain disease self-management skills and cope with transitions across care settings. Patient population ranges from young adulthood through late adulthood. Works closely with Outpatient HF Clinic staff.
  • Work with the multidisciplinary Heart Failure disease management program at Jefferson Health employing current, established guidelines for the medical management of heart failure patients. Incorporate the expertise of a multidisciplinary toward patient-centered care. Assist in the development of policies, procedures, and treatment protocols for a standardized and evidence-based approach to medical management of the heart failure patient. Work in a leadership role in the Heart Failure Task Force to define optimal systems of care delivery to improve heart failure outcomes.
  • Directly administer medical care to heart failure patients and oversee the care provided by other disciplines. Introduce newly approved treatment and diagnostic modalities and develop expert understanding and ability to prescribe and utilize new treatment/diagnostic tools. Develop high level of expertise in treatment of heart failure patients receiving device therapies such as CRT, ICD and LVAD.
  • Maintain active involvement in and responsibility for the medical management of patients across the spectrum of the Comprehensive Heart Failure Program, including the inpatient, outpatient, ER and Observation settings as well as continued management of heart failure patients transferred to partnered skilled facilities. Collaborate in an ongoing fashion with the Medical and Clinical Directors and cardiologists of the Comprehensive Heart Failure Program. Foster a good working relationship with referring physicians. On call responsibilities include telephone availability to Heart Failure VNA and hospital RN and resident staff.
