Job Description
The ambulatory care pharmacy specialist will collaborate as an integral member of the multidisciplinary team in the provision of care for patients in our medical clinics to optimize and promote rational, safe, evidence-based and cost-effective therapy. In addition to direct patient care responsibilities, the clinical pharmacy specialist will: actively participate in committees and /or workgroups; collaborate in the review of new and existing medications, clinical practice guidelines and other related policies used in the management of the clinic-specific medical conditions. This person also conducts clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice

Qualifications:

Education:
• PharmD degree from an accredited college of Pharmacy.
• Or, licensed Pharmacists prior to 2002 with Bachelor's degree from an accredited college of pharmacy.
Experience:
• PGY1 pharmacy training or three years equivalent experience
• PGY 2 residency preferred.
Licensure, Certification, Registration:
• Current unrestricted pharmacist license from Connecticut Board of Pharmacy or eligibility within 45 days.
• -Board Certification within specialty practice within 6 months of eligibility
• Ability to work collaboratively as part of a healthcare team.
• Ability to assess operational logistics and make recommendations to pharmacy leadership
• Ability to adapt H3W Leadership Behaviors and implement on a routine basis
