JOB SUMMARY- WEIGHT MANAGEMENT AND WELLNESS

The Registered Nurse (RN) works in conjunction with members of the Brigham and Women's Hospital Center for Weight Management and Wellness (CWMW). The RN works in collaboration with the inter-professional Endocrine team and department. Patients seen by this practice are adults presenting with overweight and obesity and obesity-related medical conditions including, but not limited to, Type 2 diabetes, Sleep Apnea, Hypertension and Dyslipidemia. Care provided within this setting is defined as outpatient obesity management and outpatient obesity education.

Patient care is provided onsite, may require some local travel.

NATURE AND SCOPE OF PRACTICE AND WORK SITES:

The Registered Nurse practices and facilitates the implementation of evidence-based care standards for obesity for adult patients in the outpatient Center for Weight Management and Wellness Center-Endocrinology Clinic. The nature and scope of Registered Nurse's practice includes patient and family assessment, diagnosis, development of a plan of care, evaluation of treatment and education.

The Registered Nurse works in collaboration with clinical staff (attending, fellow, house officer, physician's assistant, clinical nurses, nurse practitioners, dietitians, pharmacists) from outpatient obesity treatment in the CWMW clinics including 221 Longwood Avenue, Faulkner Hospital, Patriot Place (Foxboro) and any other BWH service requesting consultation from the CWMW medical weight management team.

Specific responsibilities include, but are not limited to:

  1. Works collaboratively with team members to provide direct care, counseling, and teaching to patients with obesity and obesity-related comorbidities.
  2. Provides comprehensive obesity education in both one-on-one as well as group settings, which may include travel among various primary care and specialty clinic sites.
  3. Provides education and advice on the management or prevention of obesity to patients who either have obesity or are at risk for developing obesity.
  4. Provides patient instruction in the use and administration of oral and injectable medications.
  5. Provides obesity care education to professional staff in primary care and specialty practices, including registered nurses, physicians, registered dietitians, and health educators.
  6. Collaborates with multidisciplinary team members to increase awareness and promote obesity education services throughout the Brigham network or care providers.
  7. Collects and reports program data to the Clinical Program Manager
  8. Participates in program development, policy development and committee work as determined by the Co-Director of the CWMW
  9. Working as part of the greater Endocrine Department nursing team.
  10. Collaborates with CWMW team members in Quality activities.
  11. Works with CWMW team members to help ensure safe transitions from ED and inpatient to ambulatory care. This may involve telephone follow-up to ensure patient safety and establishment of outpatient plan.

  3. Assesses patient's acuity accurately when making decisions regarding staffing, transfers and assignments.
  4. Demonstrates awareness of the need to manage within established budgetary boundaries.


G. Personnel

  1. Participates in the orientation of new staff members, students and others as appropriate.
  2. Provides input into the clinical evaluation of other staff, as appropriate.
  3. Assumes responsibility for the unit in the absence of leadership personnel.
  4. Adheres to hospital and nursing policies and aids in their interpretation to others.
  5. Recommends change in policies and procedures through appropriate channels.
  6. Participates in the cooperative effort and peer support required for the smooth running of the unit, e.g., flexibility in relation to patient assignments, shift assignment, or work schedule.

Qualifications

To qualify you must have:

  1. Graduate of an approved school of nursing with current registration in Massachusetts. Bachelor of Science Degree in Nursing is required for newly licensed nurses and external applicants.
  2. Minimum 3 years clinical nursing experience required.
  3. Minimum of 1 year of clinical nursing experience in an Ambulatory Clinic/Practice setting required. Experience only in an ambulatory procedural area or emergency room not sufficient.
  4. Minimum 1 year of experience with telephone triage and telephone-based clinical assessment techniques is required. This is the safe, effective, and appropriate disposition of health-related problems and includes: assessing the severity of symptoms, communicating urgent patient needs to the LIP, referral to appropriate setting or level of care, documenting interventions, and an individualized plan of care using an EMR, education of patients and families on their disease process and interventions to promote health and well-being, preliminary interpretation of data, providing home health advice and follow-up, ability to make triage decisions quickly and calmly in emergency situations, and assuring continuity of care.
  5. Experience with in-person triage and clinical assessment techniques of an adult patient within the last 4 years is required.
  6. Experience in an Ambulatory Medical Specialty (not procedural) area or Primary Care with patient care delivery as an RN is required.
  7. Internal Medicine nursing experience strongly preferred.
  8. Recent weight management experience (within last 4 years) strongly preferred.
  9. Previous experience and demonstrated skill inserting and managing peripheral IVs strongly preferred.
  10. Must be comfortable with all aspects of patient care in person and via telephone.
  11. Experience with team-based care preferred.
  12. Case Management experience preferred.
  13. Bilingual English/Spanish preferred.


Other:

  1. Membership in professional organizations is recommended and nursing certification in Ambulatory Care or an Ambulatory Specialty area is preferred.
  2. Able to work weekend rotation per the MNA contract.
