Start Date or Shifts Requested: 5/28-31, 7/1-5 (2 providers needed), 7/15-19 (2 providers needed), 8/26-30, 9/23-27
Daily Schedule: Hours 7:00 am- 3:30 pm. Hours may extend past 3:30 pm with surgery add-on cases. (no 8-hour guarantee, unless requested by provider)
Call Schedule: Call is shared with 2 CRNAs, Weekday call is 1:4; Weekday call 7:00 a.m. 7:00 a.m., Weekend call is 1:4; Friday 7:00 a.m. Monday 7:00 a.m., Post call day off
Minimum shifts/coverage to be considered: Full time is highly preferred, will consider part time
