The Addiction Consult Team (ACT) Advance Practice Provider is a position with the Massachusetts General Hospital Program in Substance Use and Addiction Services (PSAS), a hospital-wide program to improve access to evidence-based substance use disorder care. ACT is a multidisciplinary team including physicians, advance practice providers, social workers, a recovery coach, and a resource specialist. ACT provides consultation to a spectrum of patients with substance use and substance use disorders who are admitted to the hospital, including those on medical, surgical, obstetrical, and critical care units. The service focuses on providing prompt assessment, treatment initiation, stabilization, and connection to recovery support programs. This role will provide expert addiction care to inpatients seen by ACT.

This position has the option for per-diem or part-time hours, shift 8am-6pm. Weekend availability is required. There is an option to cross train and pick up per-diem work in the MGH Bridge Clinic, and outpatient low barrier addiction clinic.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Clinical Practice:

  • Provide patient assessment, diagnosis, determination of a treatment plan, withdrawal management, initiating maintenance pharmacotherapy, and evaluation/management of Substance Use Disorder
  • For APPs - works collaboratively with precepting attending physicians for evaluation/management of consults
  • Demonstrates patient-centered approach to care, engages in shared decision making with patients, recognizes their subjective health needs, past experiences, and treatment preferences. Is willing and able to engage with patients in all stages of readiness
  • Demonstrates competency in Harm Reduction as a philosophy and set of core practices and is comfortable integrating harm reduction services into clinical care
  • Demonstrates critical thinking and diagnostic reasoning skills in clinical decision-making
  • Serves as a patient advocate and facilitates compassionate and trauma-informed approach for inpatients followed by ACT


Education:

  • Opportunity to participate in activities that contribute to the education of health care professionals including presenting and publishing when applicable
  • For NPs - Opportunity to facilitate the educational experience of nurse practitioner students and fellows working on ACT


Professional Development:

  • Engage in self-performance appraisal on an annual basis, identifying areas of strength as well as areas for professional and practice development
  • Identify individual learning needs and goals and develop a plan to meet them
  • Maintain clinical knowledge and skills based upon current nursing and health care practices
  • Maintain membership in professional nursing organizations
  • Maintain the standards and educational requirements specific to the advance practice provider role and licensure
  • Maintain and update clinical knowledge and skills based on current health care practice/research


Qualifications

LICENSES, CERTIFICATIONS and/or REGISTRATIONGS

  • Current licensure as a Nurse Practitioner, Clinical Nurse Specialist or Physician Assistant in the Commonwealth of Massachusetts
  • Current DEA licensure and Mass Controlled Substances Registration
  • For NPs - Certified Addictions Registered Nurse - Advanced Practice (CARN-AP), or willingness to achieve CARN-AP certification in first 12 months of employment


EDUCATION

  • Masters Degree in Nursing required or
  • Graduate of an accredited American Medical Association-approved Physician Assistant training program required


EXPERIENCE

  • 2-4 years clinical experience required with experience treating patients with substance use disorders
