Job Description

Housing for Health (HFH) at NYC Health + Hospitals (H+H) recognizes that the chronic health issues of unhoused individuals cannot be treated without stable housing. This initiative seeks to improve the health and wellbeing of New Yorkers by focusing on four strategic areas: housing navigation services, medical respite beds, affordable housing on hospital property, and social service support for patients in permanent housing. In 2023, Housing for Health launched the Housing Location and Placement Services (HLPS) initiative to provide patients experiencing homelessness with one-on-one support to find a permanent home. The HLPS initiative contracts with community-based organizations (CBO) to provide housing support including: identifying supportive, affordable or private market housing; matching patients to appropriate permanent housing to meet their needs; helping patients complete housing applications; escorting patients to housing viewings; and supporting patients transition to housing.

Under the direction of the Senior Director of Housing Policy and Operations, the Assistant Directors will oversee the day to day operations of the Housing Location and Placement Services (HLPS) CBOs. This includes overseeing all housing placement activities for patients experiencing homelessness and the progress of the CBOs toward housing targets and goals. The Assistant Directors will coordinate with H+H systemwide referral sources during the housing search and placement process, and collaborate with city agencies to support patients towards their permanent housing goals.

General tasks and responsibilities include but not limited to:
• Oversee all day to day operations for Housing Location and Placement Services CBOs. This includes leading case conferencing to identify housing barriers and needs; supporting the team with interagency, H+H, and landlord coordination; ensuring the contracted CBO(s) are making progress towards housing targets and goals; and providing quality assurance to ensure work adheres to the contract scope of work, milestones, and goals.
• Manage the HLPS CBO(s) through all aspects of the housing process including matching patients to vacancies, overseeing housing progression, and expediting housing rent-ups.
• Works closely with the Senior Director of Housing Operations and Policy on operations for permanent housing placements including but not limited to development of templates, policies, and procedures; tracking and reporting progress towards goals; and assistance with administrative tasks
• Assist with difficult and challenging cases that can include facilitating solutions with health care teams, patients and government partners including DHS, HPD, NYCHA and HRA.
• Oversee Section 8 vouchers and other rental subsidies
• Manage supportive and affordable housing rent ups in collaboration with property managers, city agencies, and owners.
• Maintains and develops relationships with various internal stakeholders including community health workers, care coordinators, social workers and physicians providing care and supports for patients experiencing homelessness.
• Works with Care Management providers at H+H to ensure patients referred to Housing for Health are connected to appropriate care management.
• Utilizes web-based computer systems including but limited to NYCHA Portal, HRA CAPS, HOME and H+H internal computer systems.
• Serves as a team representative in intra/inter agencies trainings and meetings as required.
• Manages and assists with special projects as requested by Assistant Vice President and Director of Housing Policy and Operations.
• Performs other related duties, as directed.
• Manage other special projects as needed.

Minimum Qualifications

* Qualification for Central Office.

# Qualification for Health Care Facilities.
1. A Baccalaureate Degree from an accredited college or university in Business Administration, Public Administration, Hospital Administration, Social Sciences, Management, Health Care Administration or an approved related field in an accredited college or university; and,
2. Six* or five# years of progressively responsible planning, design and program development experience pertaining to the operation of a major segment or all of a hospital or health care facility including substantial exposure in meeting community health needs, health care planning, design and program implementation functions with at least three* or two# years of responsible level administrative experience coordination health care planning, design, development and public policy programs; or,
3. A Master's Degree from an accredited college or university in Social Science, Hospital Administration, Public Administration, Business Administration, Health Care Administration, Administrative Medicine in Public Health or approved related field; and,
4. Five* or four# years of progressively responsible experience as described in (2) with at least two years of responsible managerial and highly responsible level administrative experience coordinating health care planning, design, development or public policy programs; or,
5. A satisfactory equivalent combination of education, training and experience.

Department Preferences

Preference will be given to qualified candidates with the following knowledge, abilities, education, experience and/or skills:
• New York City residency (Brooklyn, Bronx, Manhattan, Queens or Staten Island) preferred.
• Professional and/or direct experience in handling the housing process and/or housing applications in NYC, including affordable housing, supportive housing, market rate housing and City and Federal rental vouchers.
• Ability to independently troubleshoot housing barriers
• Professional and/or direct experience working with clients experiencing homelessness, housing insecurity, and entitlements.
• Excellent written and oral communication skills.
• Case management experience.
• Proficiency in speaking a language other than English, preferably Spanish, Haitian Creole, Mandarin, Bengali, Russian, Cantonese, French, Arabic, Hindi, Urdu, Albanian, or Korean.
• Ability and willingness to travel within the five boroughs of New York City to meet patients at either current shelter residence, H+H facility and accompany apartment viewings.
