Responsibilities

The Senior Social Work Associate in the C.A.R.E. Access Program is responsible for obtaining all related medical psychosocial and benefits information through the intake process needed to assist prospective clients to access services in the clinic and or case management program that is most appropriate based on health and functional acuity.

The Senior Social Work Associate oversees clients and facilitates transfer to the appropriate case management program if their level of acuity increases. This position is responsible for the provision of an initial assessment of psychosocial needs for those clients enrolled in the social model case management program and for developing implementing monitoring and evaluating an agreed upon individualized plan of care for each client. The Senior Social Work Associate is responsible for linkage of clients to community resources and advocating for services; through professional intervention and education is responsible for assuring that services are supportive of each client maintaining a maximum level of personal control and independence.

Qualifications
  • Knowledge of HIV/AIDS and community resources required.
  • Must have excellent communication and interpersonal skills.
  • Experience in a health or human service setting.
  • AA Degree or Bachelors Degree in Social Work
  • Demonstrated experience working in the HIV field may be substituted for education


Preferred Requirements:
  • Spanish speaking ability strongly preferred
