BH Coordinator provides guidance, oversight and supervision or all Licensed Behavioral Health Providers at HCH Program of Manchester, as they engage in direct patient care and collaborate with other members of the HCH team to address mental health, substance use disorder and concrete service needs of patients. BH Coordinator provides guidance, oversight and supervision as team leader for Street Outreach Team, Medication Assisted Treatment program, Outreach/Enrollment efforts, and Health Education endeavors to ensure effective operations and adherence to program requirements. In conjunction with the Medical Director and the Program Coordinator ensures highest quality of clinical care and achievement of clinical performance outcomes.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Under general direction of HCH Medical Director and Program Coordinator, and within established policies and procedures of HCH Program and Catholic Medical Center, the incumbent performs the following functions:

1. Supervision & Quality Management:
  • Oversee and supervise BH providers as they engage in direct patient care, serving as professional resource day to day, conducting supervision sessions for each provider on a regular basis, and completing annual performance appraisals as required.
  • Provide supervision towards State of NH Mental Health (LICSW/LCSW, LCMHC) and/or MLADC licensing to appropriate, license-eligible Behavioral Health staff members.
  • Oversee and supervise Community Health Workers, Recovery Coaches, Outreach / Enrollment staff and Health Educators as they perform as members of the HCH team within clinic, outreach and health education settings; complete annual performance appraisals as required.
  • In conjunction with Medical Director and Program Coordinator conducts ongoing evaluation of entire HCH program to identify practice issues, implement corrective actions, improve systems, and create policies and procedures that ensure high quality of patient care and excellent program performance. Participates in long term strategic planning to identify goals and priorities, in line with the mission and values of HCH program.
  • Promote integration of BH care within all HCH clinical, outreach and health education efforts; ensure achievement of BH care performance outcomes in all of these efforts, providing consultation and guidance to members of HCH team, at clinical case management meetings.
  • Provide in-service education about BH care for all members of HCH team, as needed.
  • Participate as a member of the HCH QI committee to evaluate progress toward achievement of BH clinical performance outcomes, helping to devise systems and corrective actions that support progress toward achievement of goals.
  • Assist with grant writing and reporting for MAT and Behavioral Health endeavors, in conjunction with the Program Coordinator.
2. BH Counseling:

• Conduct BH screenings and assessments of patients, in collaboration with HCH medical team, to implement prompt, effective BH interventions during Triage and Open Access clinic sessions; receives warm-hand off patient referrals from medical team while serving as on-call BH provider at a given clinic session.

• Perform screenings for all HCH patients, to identify mental health, substance use disorder, concrete services, and age-specific needs of patients. Conduct psycho-social assessments with patients, as appropriate, based on screening findings.

• Formulate individualized BH care-plans based upon identified needs of patients, and recommend appropriate levels of care aligned with evidenced-based standards of practice from sources such as DSM-5 and American Society of Addiction Medicine.

• Collaborate with other members of the HCH team to formulate, discuss and carry-out care-plans, to guide patients toward recovery and self-care management.

• Provide ongoing counseling, recovery support, relapse prevention, life skills training, vocational counseling, crisis intervention, suicide prevention, and psycho-education training as indicated, to patients in individual, couple, and/or group settings.

• Coordinate patient referrals to Mental Health, SUD and Respite care providers, programs, and treatment facilities in the community, as indicated.

• Document all BH screenings, assessments, and interventions in EMR, in accordance with QI Plan objectives, HCH program expectations for ICD-code claims submissions and UDS encounter utilization data tracking, and in accordance with CMC policies and procedures.

• Respect the dignity and confidentiality of all patients abiding by HIPPA standards, federal disclosure laws and policies related to patient confidentiality and privacy, in all verbal, written and electronic documentation and communication related to patient care.

3. Concrete Services:

• Assess and address socio-economic needs of HCH patients; promote access to supportive housing, public entitlements, employment, other health and human services and other community resources, as indicated.

• Assist patients with access to services and resources, coordinating referrals and placement, as needed.

• Collaborate with Hospital and ER staff for discharge planning of homeless patients.

• Collaborate with local community health and human service providers and partner agencies, on behalf of homeless patients, promoting access to care and enrollment into HCH program.

Qualifications

Education:

• Master's Degree in Behavioral Health, from an accredited program

Experience:

• Minimum of two years' experience in Behavioral Health, Outpatient or Community Health setting

Licensure / Certification:

• LCMHC/LICSW in State of NH & MLDAC
