Bilingual Nurse Care Manager (Hybrid)

Medix is looking for a self-motivated and passionate RN for our Hybrid Bilingual Care Manager opening. This role will consist of assessments, care coordination, monitoring patients services and setting up covered servcies within the Geriatric/MLTC community. The candidate must be bilingual speaking in Russian or Spanish.

Duties:

• Case management, care planning, coordination, and assessment of health.
• Monitor services for quality, effectiveness, and progress toward goals.
• Coordination for patients, discharge planning from hospital or nursing home.
• Identifies health, environmental, and psychosocial risks for members, recommends appropriate interventions to the Interdisciplinary Care team.

Specifics:

Hours: Mon-Fri 9am-5pm

Job Type: 1-2 days in the field a week (Brooklyn) 3-4 days remote!

Pay: $95,000 - $115,000

Requirements:

• NYS RN License and 3+ years experience in one of the following (MLTC, case manamgent or working with the geriatric community)
• Bilingual speaking (Spanish) or (Russian)
• BSN
• Passionate attitude, communication skills and problem solving will be vital to this position.
