• Certification (Valid California Special Driver Certificate with PS Endorsement)
• CPR/First Aid Certification (Valid CPR and American Red Cross First Aid Certificates)
• DMV Printout (K4 Report (within 30 days))

• Health (Medical examination card)
• Letter(s) of Recommendation (one letter dated within the past year)
