The primary responsibility of the Care Advocate is to identify, track, monitor, and coordinate the care of patients enrolled in Care Management programs. As a Care Advocate, you will act as a resource and advocate for patients and their families to ensure an optimal level of health. This position is responsible for communication to patients, physicians and other care management team serving as a liaison with external departments to ensure optimal customer service.

This position is full-time, Monday - Friday. Employees are required to work our normal business hours of 8:00am - 5:00pm CST. It may be necessary, given the business need, to work occasional overtime or weekends. Our office is located at 909 Hidden Ridge Irving, TX.

We offer weeks of on-the-job training. The hours of the training will be 8:00am - 5:00pm CST, Monday-Friday.

Primary Responsibilities:

  • Establishes a trusting relationship with identified patients, caregivers, clinic staff and physicians.
  • Keeps patients actively engaged with their primary care physician and assists patient with any scheduling medical appointments/transportation.
  • Partners with the patients care team (community, providers, internal staff).
  • Serves as primary point of contact for internal and external stakeholders.
  • Provides guidance and inquires if patients need help with any specific barriers to healthcare through telephonic outreach to assist patient's compliance with their care plans.
  • Identifies health plan resources or local community resources to provide solutions for the patient.
  • Engages patient, family, and caregivers to assure that a well-coordinated treatment plan is established.
  • Creates referrals to outside source(s) to assist with patient's self-management goals.
  • Supports the patient to ensure pick-up and adherence of their medications.
  • Proactively engages the patient to manage their care.
  • Liaisons with providers and facilities telephonically, to assist in problem solving regarding patient needs.
  • Communications with internal staff, patient/caregiver and community providers must be accurate and timely.
  • Patient outreach using compassionate customer service with the goal to inform them of care gaps and facilitate the closing of those gaps.
  • Documents all patient interactions in a concise manner that is compliant with documentation requirements for Model of Care and Center for Medicare and Medicaid Services (CMS) regulations.
  • Conducts in-bound and out-bound calls including, but not limited to: patient scheduling, surveys/screenings, census management and distribution of materials to appropriate clinical personnel or patients.
  • Provides clerical and/or administrative support to clinical staff and managers.
  • Enters timely and accurate data into the electronic medical record to communicate patient needs and ensure complete documentation of patient visits and phone calls.
  • Performs all duties for internal and external customers in a professional and responsible manner.
  • Adheres to organizational and departmental policies and procedures.
  • Performs all other related duties as assigned.

Required Qualifications:

  • High School Diploma / GED OR equivalent years of experience
  • Must be 18 years or older
  • 2+ years experience in a physician's office, clinical/hospital setting, or Medical insurance company
  • Experience communicating productively with individuals and within multidisciplinary teams
  • Experience with windows based programs including Microsoft Word, Excel, Access and Outlook
  • Ability to work our normal business hours of 8:00am - 5:00pm CST. It may be necessary, given the business need, to work occasional overtime or weekends.

Preferred Qualifications:

  • 3+ years of experience working in a physician's office, clinical/hospital setting or Medical insurance company
  • Bilingual fluency in English / Spanish
  • Experience working with Special Needs Population
  • Basic understanding of Medicare and Medicaid health plans
  • Experience working with psychiatric and geriatric patient populations

Telecommuting Requirements:

  • Reside within 100 miles from 909 Hidden Ridge Irving, TX
  • Ability to keep all company sensitive documents secure (if applicable)
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy.
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.
