You Will Be Responsible For:

Monitors performance of the staff and reports findings to Care Management Leadership or takes corrective action for improvement as appropriate.

• Utilizes critical thinking skills in the performance of their duties. Supports the planning, organizing, and leading of the team through application of critical thinking skills. Utilizes data-driven decision making, incorporating evidence into the decision process.

• Performs skills necessary to create a high-quality patient experience, as reflected through acceptable satisfaction scores, quality monitoring, and patient feedback.

• Identifies, documents, and tracks avoidable delays.

• Demonstrates appropriate customer-care skills such as empathy, active listening, courtesy, politeness, helpfulness and other skills as identified.

• Performs skills necessary to create a high-quality patient experience, as reflected through acceptable satisfaction scores, quality monitoring, and patient feedback.

• Copies, scans, files, and distributes documents.

• Documents activities and progress in the electronic medical record per defined policy and procedures, computer billing/utilization systems, or in other ways as directed. Inputs clinical information and discharge planning details into the medical record as appropriate for the role.

• Keeps RN Care Manager, Social Worker and/or other members of clinical team informed of issues or delays impacting a timely discharge or scheduling of post-acute appointments.

• Performs other duties as assigned.

Additional Duties and Responsibilities for Care Connector Lead assigned to the post-acute team:

• Prepares documents and reports required data for clinical reviews and discharge plans for third-party payers and other internal and/or external groups.

• Contacts third-party payers to obtain and/or confirm authorization for services and communicates necessary related clinical information, when required.

• Assist in the completion and maintenance of care management contracts during and after patients hospitalization.

• Ensures all contract information is accurately documented and maintained in patients medical record.

• Thoroughly researches issues and takes appropriate action to resolve them within turnaround time requirements and quality standards. Escalates issues as appropriate.

Additional Duties and Responsibilities for Care Connector Lead assigned to the acute care and/or ED team:

• Delivers and appropriately documents delivery of regulatory letters to maintain compliance with CMS/AHCA regulations.

• Communicates with Care Management team regarding regulatory compliance.

• Coordinates and schedules post-acute follow up appointments with physicians, testing, and other services when appropriate. Provides verbal and written confirmation of appointments to patients/families. Documents appointments and the patients/families agreement and understanding of schedule in the medical record.

EDUCATION AND EXPERIENCE REQUIRED:
  • High School Diploma or GED
  • At least two-years of experience in a customer service environment
  • At least two-year experience in a healthcare environment

EDUCATION AND EXPERIENCE PREFERRED:
  • Associate's Degree in a healthcare-related field
  • Experience working in a healthcare setting
