In this critical role, you'll conduct assessments, develop recovery plans, and provide services based on Trauma-Informed and Patient Centered Models. You'll coordinate services, monitor progress, and deliver interventions to support our patients on their recovery journey. Your Role in Action

As a Care Coordinator for this unit, you will be:
  • Dispatch, as part of a two-person team, to calls for service in the community and provide immediate
    and effective support and intervention to individuals experiencing emotional distress, trauma, or other
    crisis situations, utilizing established protocols and best practices.
  • Offer emotional support and guidance to individuals in crisis to help manage their emotions and cope
    with the situation effectively.
  • Collaborate with licensed clinicians and other team members to develop crisis intervention plans
    tailored to the individual's needs.
  • Coordinate with other agencies and emergency services to ensure a coordinated response to incidents
    requiring emergency intervention, communicating quickly and effectively in emergency situations.
  • Maintain accurate and up-to-date records of crisis intervention activities, including critical event
    reports, radio communications, screening and assessments, and progress notes, ensuring compliance
    with organizational standards and under the guidance of a licensed clinician.
  • Engage in continuous training and professional development to remain up to date on the latest
    techniques in crisis intervention, emergency preparedness, crisis response protocols, and safety
    measures. This includes staying informed about radio technology and implementing best practices for
    clear communication with first responders during high-pressure scenarios.
  • Prioritize self-care and resilience-building activities to maintain personal well-being and professional
    effectiveness during high-stress situations, seeking support and utilizing available resources as needed.
  • Uphold ethical standards and legal guidelines in the provision of crisis intervention services.
  • Assist in responding to emergency disasters by providing crisis intervention services under a licensed
    clinician's guidance. Support individuals impacted by the event, assess their mental health needs, and
    help implement crisis management strategies to promote resilience and post-event recovery.
  • Maintain and operate radios following established protocols and procedures for radio communication.
  • Flexibility to work irregular hours which may include evenings, weekends, holidays, and during
    emergencies and disasters.
  • Perform additional responsibilities, as directed by leadership, efficiently and effectively with flexibility
    and adaptability

What qualifications you will need:

Education :
  • Bachelor's degree (B.A.) from four-year accredited college or university with a major in a social, behavioral, or human service field, including but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, and educational psychology, required.

Required Experience:
  • Direct experience working in a mental health or related setting providing care to individuals including
    assessing situations, offering support, and connecting them with appropriate resources.
  • Demonstrated ability to offer emotional support and guidance to individuals in crisis, helping them
    manage their emotions, cope effectively and/or seek a higher level of care.

Other Key Qualifications
  • Ability to pass a thorough background check and meet all Criminal Justice Information Services (CJIS)
    eligibility criteria. This includes undergoing periodic audits and training to maintain clearance status.
  • Certify and maintain status as an Agency Certified Driver. This includes the ability to operate a motor
    vehicle and pass agency performed road test. The employee must possess a valid Texas State issued
    driver's license and be insurable on the agency vehicle insurance policy.

Knowledge, Skills & Abilities:
  • Proficiency in conducting risk assessments and developing safety plans.
  • Understanding of crisis response protocols, assessment tools, and trauma-informed care.
  • Familiarity with evidence-based crisis intervention techniques and mental health treatment.
  • Knowledge of mental health conditions, co-occurring disorders, and crisis prevention strategies.
  • Awareness of legal and ethical guidelines in mental health care and community resources.
  • Basic understanding of psychotropic medications, cultural influences, and effective communication
    techniques.
