Job Summary

The Care Management Assistant will provide support to the Care Management and Population Health team. This position will collaborate through direct patient outreach, communication with network organizations, and post-acute providers to coordinate care. The Care Management Assistant will document all care management activities in the approved care management system.

Responsibilities

Provide administrative support to providers, patients, and staff to expedite patient flow, telephone communications, and quality patient care. Duties will include: • Conducts patient screening for social determinants of health, identifies community resources, refers patient for resources, and conducts follow up • Assists Care Coordinators with Transitional Care Management, Utilization Management, and Chronic Care Management through patient identification, screening utilizing various information system platforms. • Assists Care Coordinators with arranging durable medical equipment, home health care, scheduling medical appointments, and pharmacy services for the patient population • Tracks and conducts follow up for different patient populations utilizing various reports and information system tools • Reinforces patient education using approved resources and tools as directed by the Care Coordinators • Recognizes and flags clinical issues to care coordinators and manager that need further assessment and intervention • Assists the Population Health and Care Management team by engaging patients in home medication reviews and reminders • Provides support to our network practices with patient outreach, scheduling, and medical record review to improve our quality performance • Maintain timely and comprehensive documentation in the approved system for all activities • Support practice team with medical record abstraction, printing, copying, and submission to approved entities • Strong customer service, verbal, and written communication skills • Ability to utilize Microsoft Office and web-based platforms • Experience with medical terminology required • Experience with electronic health records preferred • Ability to multi-task various job duties in a day

• High School Diploma or GED (Required) Education: Post High School Diploma or certificate program as Medical Assistant or Nursing Assistant Experience: at least 12 months of experience as a medical assistant or nursing assistant is preferred
