Position Summary:

The Care Management Assistant supports the licensed social worker or the nurse case manager with their transition planning activities. The Care Management Assistant is an integral member of the care management team and will work directly with patients, families, and vendors to set up services for implementing the plan of care. These activities include referrals and data entry into the Navihealth application for post-acute placements, transportation arrangements, scheduling post-discharge appointments for primary care, specialists, or clinics, organizing and maintaining consent forms, and clerical activities as needed.

Shift: Full-time, 40 hours per week,

Knowledge, Skills, and Abilities:
  1. The Care Management Assistant collaborates with the social worker/case manager to facilitate appropriate placement in rehabilitation facilities or another alternate level of care.
  • Develops working knowledge of services offered by various facilities.
  • Meets with the patient/family to discuss recommendations for rehabilitation facilities, offers choice, and enters referrals into Navihealth application.
  • Obtains/confirms payment or authorization information.
  • Confirms acceptance to facilities and communicates this to patient/family and team.
  • Consult with the social worker/case manager for any barriers or issues related to facility choice, facility availability of beds, or other issues to resolve the issue and ensure a smooth transition for the patient.
  • Documents work in the EMR according to the template.
  • Demonstrates ability to obtain care through Telligen and completes the PASSR screen for post-acute placement.
  • After discharge, provide the information in the discharge packet to accompany the patient to the facility.
  1. The Care Management Assistant works directly with the patient and family to ensure that any post-discharge appointments will meet the clinical needs directed by the provider, social worker, or case manager and fit into the patient/family's schedule.
  • Obtain appointment information- provider name, timeframe for appointment, and reason for work.
  • Meet with the patient/family to explain the type of appointment/s needed, the reason, and the timeframe.
  • Arrange appointments that meet the patient/family's expectations within the timeframe.
  • Offer to do a warm transfer between the Care Management Assistant and the provider's office to make the appointments to encourage self-management and help the patient learn the process.
  • Document the appointment information, including provider name, phone number, address, and date/time in the EMR.
  1. The Care Management Assistant will arrange transportation by the transition plan.
  • If the patient is going to a facility, will set up transportation to include the date/time for pickup.
  • Set up transportation to home via cab through the Social Work office.
  • Obtains/confirms payment or authorization information.
  • Documents any activities in the EMR.

Education: High School Diploma required. An associate's degree in health science or a closely related field is preferred.

Knowledge: Work requires the ability to exchange information on factual matters, schedule appointments, greet visitors, explain medical center policies, and relay messages to appropriate personnel. This type of interaction requires courtesy and tact when dealing with patients, visitors, and medical center employees. More sensitive situations may be referred to others as appropriate.

Work experience: A minimum of one year of previous hospital or long-term care experience, working in case management and admissions.
