The Care Management Associate is responsible for ensuring appropriate communication with third party payers to authorize hospital days. They obtain information from clinical reviews done by the case manager as well as patient medical information from other computer applications utilized by the facility.

Responsibilities
- Utilizes case management software to generate work lists
- Submits clinical information per third party payer requirements
- Upon identification of possible concurrent denial, forwards information to appropriate case manager
- Notifies third party payer of patient discharge and secures all days
- Assists with non-clinical functions to coordinate care transitions
- Utilizes effective communication skills to accurately complete work
- Collaborates effectively with all department staff to achieve goals and objectives
