The Complex Care Center (CCC) offers primary care for children with complex chronic medical conditions. The patients of the Complex Care Center have multiple diagnoses and frequently are technology dependent, needing home ventilation or tube feedings for daily functioning and quality of life. We see children from birth through adolescence, whether your child is sick, needs shots or a check-up. Our health care team is specialized in treating the most complex and chronic medical issues, in addition to preventative health visits.

Position Overview: The RN Care Manager at Cincinnati Children's plays a crucial role in addressing the coordination needs of medically complex patients receiving Primary Care at the Complex Care Center. This role is centered on education, health promotion, and increasing self-management to improve patient outcomes and overall well-being.

Shift/Hours: Days /36 hours per week (.9FTE) - Monday to Friday (8 hr shifts/ 72 hours every 2 weeks)

Expected Salary Range: Minimum $37.80/hour; Mid-point $47.89/hour; Salary determined based on years of nursing experience.

Key Responsibilities:

  • Caseload Management: Manage a caseload of patients, primarily in the outpatient setting, involving ongoing assessment, coordination, and monitoring of medical and social determinants of health (SDOH) needs.
  • Continuity of Care: Oversee and collaborate with the inpatient medical and Care Management team when patients from the caseload are admitted, ensuring a seamless transition between outpatient and inpatient settings.
  • Care Coordination: Coordinate both primary and subspecialty care at CCHMC, as well as connecting patients and their families to community resources such as DDS (Developmental Disabilities Services), school supports, Perlman Center, home care services, and waiver services.
  • Family Support: Actively collaborate with various entities to support families in maintaining stability in the home, identifying and implementing strategies to promote self-management and enhance the overall quality of life for patients and their families.
  • Advocacy: Serve as an advocate for patients and their families, ensuring they have access to necessary services and support.
  • Patient visits at Burnet and Liberty locations


Qualifications:

  • Registered Nurse (RN) with a valid nursing license.- BSN or MSN, 5-7 years exp, MSL within 3 months, and CCM within 2 years of hire or will accept CNL (Clinical Nurse Leader).
  • Experience in pediatric care coordination/Care Management, particularly with medically complex patients.
  • Strong organizational and communication skills.
  • Ability to work collaboratively with a multidisciplinary team.

Qualifications:

EDUCATION/EXPERIENCE

Required:

• Bachelor's degree in a related field

• 5 - 7 years of work experience in a related job discipline.

• Active Ohio RN License- must obtain MSL within 3-6 months of hire

• ACEN/CCNE accredited BSN OR MSN

-CCM certified or must obtain within 2 years of hire.

Preferred:

CCM certification

Pediatric care coordination experience, particularly with patients with complex medical need
