Job Description:
To partner with the Registered Nurse Care Coordinator (RNCC), Registered Nurse Case Manager (RNCM) and Social Worker (SW) to help patients get the support and resources they need across the care continuum, the CTA facilitates communication and works with nursing homes, and home health agencies, internal UAB Health System support, community based organizations, prescription assistant programs, etc to ensure the patient's successful transition across the care continuum. Typical day-to-day responsibilities include assisting with clerical and non-clerical tasks such as completing paperwork, filling out patient assistance program applications, arranging post-acute transportation and lodging needs, coordinating resources for patients, and ensuring that CMS Conditions of Participation (CoP) for discharge planning are met.

Qualifications
