JOB SUMMARY

Under the direct supervision of a registered pharmacist and following established policies and procedures, provides technical support for pharmacists and clinicians to ensure safe and effective transitions in care at admission and discharge to support medication reconciliation and discharge pharmacy services. Collaborates with patient care team. The Care Transitions Technician will employ a variety of strategies appropriate for the patient/family/situation to obtain, update, and document an accurate medication list in the patient's electronic health record (EHR). Participates in medication records review, patient profile review, staff development, and educational programs. The technician will utilize electronic tools available (e.g. external prescription history and hospital electronic medical record system) and conduct patient interviews to enter the patient's current medication list into the electronic medical record. Able to perform all of the duties of a registered pharmacy technician under the direct supervision of a registered pharmacist. Responsible for performing job related duties in accordance with the mission, vision and values of Tampa General Hospital.

This position will be working in a specialty clinic setting and the work schedule is M-F day shift.

Qualifications

  • Registration with the Florida Board of Pharmacy
  • High School degree or General Equivalency Diploma (GED)
  • Four years of pharmacy experience, community pharmacy experience preferred
  • PTCB technician certification required
  • In the absence of a current State of Florida, Board of Pharmacy issued permit as a Registration Pharmacy Technician, a current/active Pharmacy
  • Intern permit, issued by State of Florida, Board of Pharmacy may be substituted.
