Responsibilities:
  • Assisting residents with ADL tasks as indicated in their service plans. This includes but not limited to: assisting with bathing, grooming, toileting, dressing, oral care, ambulation, transfers, serving and assisting with meals, feeding, housekeeping, bed making, and laundry
  • Always observe and uphold resident rights
  • Positively supports residents with emotional and behavioral issues
  • Assist other staff as needed on the floors
  • Assist residents to and from meals
  • Supports resident activity participation by assisting residents to/from activities, encouraging participation, and leading activities when asked.
  • Must be willing to work with staff from all departments to ensure quality of care for residents
  • Keep environment safe and clean for residents; performing housekeeping duties as indicate on task sheets.
  • Observe residents for safety and report any and all changes in resident condition effectively, accurately, and in a timely manner to LN on duty
  • Assist in development and revision of Service Plans by informing supervisor of current resident needs and preferences as well as successful service approaches.
  • Answers all resident pendant calls immediately.
  • Carry and possess a working knowledge of community call system, policies and procedures.
  • All other duties as assigned.

  • Relate to residents in a courteous, empathetic, and cooperative manner.
  • Must be a mature individual. Must possess the ability to multi-task in a fast-paced environment and perform a variety of tasks, often changing assignments on short notice.
  • Must have strong organizational skills, as well as the ability to prioritize and meet deadlines.
  • Must be detail and team-work oriented.
  • Must attend required in-services/ classes to further education of skills and state requirements.


Qualifications:
  • Must be Drug free
  • Must be able to pass Criminal Background Check and TB test.
  • Must be eighteen (18) years old.
  • Maintains current CPR and first aid certification or willing to obtain.
  • Ability to read, analyze, and interpret care plans.
  • Assisted Living and Memory Care regulations familiarity.
  • Ability to prioritize and organize assignments.
  • The employee shall be responsible for maintaining the integrity of Fields Senior Living


Physical Demands and Requirements:
  • Maintain ability to lift 50 pounds; to stand or walk throughout the vast majority of the work day; and to frequently push, pull, bend, twist and reach.
  • Must be willing to be active and on feet during entire work shift.
  • Involves light to heavy physical activity performing non-strenuous and strenuous activities.
  • Must have sufficient manual dexterity to work with the fingers and to reach/hand items.
