Position Overview:

As a Medical Social Worker, you will be an integral part of our patient-centered approach to healthcare. Your primary responsibility will be to provide comprehensive psychosocial support and counseling services to patients and their families. Working closely with our healthcare teams, you will address the emotional, social, and environmental factors that impact patients' health and recovery.

Key Responsibilities:

• Conduct psychosocial assessments to identify patients and families' needs and concerns.
• Provide counseling and emotional support to patients facing medical challenges, chronic illnesses, or end-of-life care.
• Collaborate with healthcare teams to develop and implement individualized care plans.
• Facilitate support groups and educational programs to address patients' and families' specific needs.
• Assist patients and families in accessing community resources, financial assistance, and support services.
• Advocate for patients' rights and ensure they have a voice in their care decisions.
• Participate in discharge planning to ensure a smooth transition to the appropriate level of care.
• Maintain accurate and up-to-date documentation of patient interactions and progress.
