Summary:

The Case Management Assistant position serves as an extender for the Case Management team and professional staff facilitating non-complicated Home Health, Durable Medical Equipment, Skilled Nursing, by triaging non-complicated specialty clinics, and other duties as assigned. This position requires a high level of interpersonal and critical thinking skills, as well as someone who is able to work autonomously, with attention to detail and follow through.

Responsibilities:

Works under the direction of Case Management RN/ Social Worker and interdisciplinary team to facilitate non-complex sequencing & discharge planning for patients across the life span.

Seeks methods to streamline processes and is aware of hospital resources. Comprehensive knowledge of program requirements, utilization management, length of stay and factors that affect YRMC

In collaboration with care team, facilitates and coordinates serves including but not limited to: Post-discharge services of skilled nursing facility placement; including interval Mental Health Acute Care Transfers, along with communicating resources, durable medical equipment, and home health services with resources available - commercial insurance, Medicare, self-pay.

Coordinates non-complicated specialty clinics under the supervision of the RN to include clinic triaging & post clinic follow-up. Assists in arranging outpatient wound care, dialysis and other patient discharge needs. Knowledge of processes for charity services including medication distribution, durable medical equipment and transportation needs.

Demonstrates knowledge and competency of department policies and standards of practice and adheres to regulatory requirements. Documentation is completed timely and meets regulatory guidelines.

Provides support, remains flexible with assignments as needed. Ability to function autonomously, while remaining under the direction of the Case Manager, Registered Nurse, or Social Worker

Competent skills in data entry excel spreadsheets, creating basic reports. Seeks out opportunities to support the department with other organizational initiatives.

Receives processes and distributes insurance reviews, prior authorization requests, and/or enters authorization information into EPIC work queue. Perform other duties as assigned.

Education:
Essential:
* HS DIPLOMA/GED

REQUIRED:
High School Diploma, GED, or equivalent and 2 years of experience in Healthcare.

PREFERRED: Licensed Nursing Assistant in Arizona. Will substitute 2 years of clerical healthcare setting experience for Patient Care experience. Completion of college level coursework. Previous healthcare experience in an adult acute care or case management setting. Experience with EPIC Electronic Health Record or similar electronic record systems.
