MedStar Health is looking for a Care Management Assistant t o join our team at M edStar National Rehabilitation Hospital. We are looking for someone with t yping and filing skills , as well as w orking knowledge of word processing, spreadsheet, database, and Microsoft PowerPoint software applications.

The Care Management Assistant will provide administrative and other support to the C ase Managers and Clinical Social Workers.

Primary Duties:

  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.
  • Participates in multidisciplinary quality and service improvement teams as appropriate . Participates in meetings, serves on committees, and represents the department and hospital/facility in community outreach efforts as appropriate .
  • Delivers patient notification letter as needed----Medicare Important Message, HINN and others. Obtains the appropriate signature and uploads into the electronic record.
  • Escalates issues to the department manager/director as needed.
  • Assists Case Management team with post-Acute care placement (Skilled Nursing, Hospice, Dialysis, Home Health, DME, Long-Term Care, and LTACH placements). Responsible for following through until discharge.
  • Obtains authorizations for post-acute care (Skilled Nursing, Hospice, Dialysis, Home Health, DME, Long-Term Care, and LTACH placements) and communicates information between the hospital, payor, and post-acute facilities.
  • Coordinates patient transport. Arrange transportation, provide documentation, track data, and follow up to confirm successful transport.
  • Participates in department projects and meetings.
  • Prepares discharge packets to be sent with patients being discharged to skilled facilities.
  • Promotes customer satisfaction through effective communication at all times.


Qualifications:

  • High School Diploma or GED. Bachelor's Degree, Preferred.
  • Medical terminology knowledge .
  • 1-2 years of related experience, Required .
