General Summary:
  • Coordinates post discharge referrals and placement for complex patients with multiple discharge barriers
  • Coordinates patient care services and resources to assure optimal clinical quality and financial outcomes as patient moves to the community from the hospital setting
  • Works collaboratively with Utilization Review Nurses, physicians, physical therapists and other clinical team members

Minimum Qualifications:
  • Master's Degree in Social Work, LCSW preferred
  • Licensure/Certification/Registration: Current Maryland licensure as Social Worker required; Certification in case management desirable
  • Experience in a clinical setting as a hospital discharge planner


Salary Range: Minimum $40.70 per hour - Maximum $40.70 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
