Case Management Registered Nurse - Case Management

TGH Crystal River is a 128-bed Joint Commission-accredited acute care hospital which opened in 1978. In keeping with their heritage, the hospital provides quality, compassionate healthcare to residents of Citrus County and surrounding areas. TGH Crystal River care services include a dedicated orthopedic bone and joint center, cardiac catheterization lab and freestanding emergency department in Citrus Hills. Surgical services include robotic-arm assisted, minimally invasive options for general and bariatric surgeries. TGH Crystal River is designated as an Accredited Chest Pain Center with PCI (Percutaneous Coronary Intervention), and Accredited Heart Failure Center and is a Joint Commission Certified Primary Stroke Center.

• Active FL. RN License

• 2 years acute care experience and 1-2 years Case Manager (CM) Experience

• CM experience preferred at an acute care hospital

• Discharge Planning Knowledge

• Facilities a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual's and family's comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.

• Enhance the quality of patient management, maximize satisfaction, and promote cost effectiveness.

Qualifications

• Active FL. RN License

• 2 years acute care experience and 1-2 years Case Manager (CM) Experience

• CM experience preferred at an acute care hospital
