Position Summary
The Case Management Specialist functions as a member of the discharge planning team or Post Visit Call team assigned to coordinate the care and services for assigned patients for an episode of illness or treatment in collaboration with the patient, family, physician, patient care team, and payers. The Case Management Specialist utilizes communication, organizational, and problem-solving skills to implement the post-hospital care plans and interventions defined by the RN Discharge Coordinator/or hospital discharge information available to them from the patients' electronic medical record (EMR). The Case Management Specialist promotes and evaluates the effective utilization of resources using current knowledge and awareness of community resources and assumes a pivotal role with the patient care team to achieve optimal clinical and resource outcomes.
Qualifications
Education

  • Associates degree in business, healthcare administration or related field, or the equivalent in education and experience is required.
  • Bachelor's Degree in business, healthcare administration, or related field, preferred .

Experience

  • At least 3 years of experience coordinating care in areas such as acute care, skilled nursing, home health setting, or similar areas where effective communication and administrative functions are demonstrated is required.
  • Experience coordinating with community resources and demonstrated knowledge in applying specific criteria for accessing those resources is required.
  • Experience with Microsoft Word and Excel required and experience with EPIC, preferred
  • Current understanding of CMS regulations related to hospital discharge requirements, preferred .
