Job Description Summary
The Case Manager Assistant is responsible to assist in the administration of the activities of Utilization Review and Discharge Planning. Responsibilities may include arranging home health services or home programs, DME, other post-acute care (outpatient dialysis, SNF referrals, etc) at the time of discharge. Exemplary customer service is a top priority, as successful, professional relationships with post-acute providers and agencies will be instrumental in this role’s success.

Education:

High School Diploma required

Experience: 1 - 3 years healthcare experience preferred but not required
