As an important member of the multidisciplinary care team, the Case Manager acts as a patient advocate by coordinating the care and service of patient populations across the continuum of care; promote effective utilization and monitoring of health care resources by collaborating with the health care team; maintain accurate knowledge of federal, state and payer standards; and assume a leadership role with the interdisciplinary team to achieve optimal clinical and resource outcomes. Core processes include risk assessment and management of clinical populations, discharge planning and utilization management.

Moffitt's Case Management Department is looking for a Full Time RN Case Manager to join our team!
Position Highlights:
- The Case manager coordinates the care and service of selected patient populations across the continuum of care; promote effective utilization and monitoring of health care resources; maintain accurate knowledge base of payer information for team collaboration; and assume a leadership role with the interdisciplinary team to achieve optimal clinical and resource outcomes.
- The case manager represents department in meetings with senior administration and provides enhanced expert services to all departments with the organization.
- The case manager demonstrates ongoing leadership within the hospital, community and professional organizations as required.
The Ideal Candidate
The ideal candidate will have the following qualifications:
- Recent experience within Utilization Review, Quality Management, and/or Case Management
- Demonstrated experience in oncology nursing Preferred
- Computer-literate, with knowledge of cost accounting and financial data analysis, Medicare and Medicaid regulations, third party reimbursement, medical record coding and pricing of services
Responsibilities:
- Discharge planning for inpatients
- Utilization Review for inpatients
Education Level:
Bachelor's Degree in Nursing Preferred
- Associates Degree in Nursing plus 1 year of relevant Acute Case Management experience may be considered.
Licensure and Certification:
Minimum 5 years clinical experience in an Acute Care setting required.
Must be able to work independently with minimal direction, anticipate and organize workflow, prioritize, and follow through on responsibilities.
- BSN or MSN Preferred
- ACM or CCM Certification preferred
- Demonstrated experience in oncology nursing preferred
- Extensive knowledge of nursing care, healthcare trends, community and healthcare resources preferred.
- Knowledge of state and federal regulations governing discharge planning, utilization management, and other regulatory agency requirements by the Joint Commission and CMS strongly preferred.
- Working knowledge of InterQual criteria preferred Strong interpersonal communication skills preferred.
- Competent computer skills to include Cerner and Microsoft Office products preferred.
