Case Manager (Nonclinical)

We are hiring a Neuro-Rehab Case Manager in Woburn, MA on a Hybrid basis!

This is a unique opportunity where the Case Manager goes into the office 2 times a month, works from home, and travels one a month out of state to see a client. All travel expenses are paid for!

Job Specifics:

Hours: Monday-Friday FT 8am-4pm; 9a-5pm

Location: Hybrid (Office in Woburn, MA)(Travel once a month out of state-paid for)

Salary: $55,000 - $65,000

Job Type: Direct hire (full medical benefits, PTO and retirement plans included)

Job Summary/Duties:

The Case Manager is responsible for coordinating and monitoring community-based services to support
veterans in the community. The goal is to help veteran's increase their independence quality of life!

• Conduct assessments to determine needs and offer recommendations for services.
• Conduct research to identify services (direct support, home health aide and/or alternative therapies), support networks and potential funding options to address assessment recommendations.
• Maintain monthly contact with clients, community providers, and funding sources.
• Document case notes, monthly summaries and files.

Qualifications:

• Bachelors degree in health care or related field (rehabilitation counseling, social work, speech and language
  therapy, occupational therapy, psychology, education, veterans' services, social services, public health, etc)
• 2 years of Case Managemenrt experience
