Great opportunity to join an experienced and collaborative team covering social work in a large fast paced Emergency Department.

Medical/ surgical social work focus

Role consists of screening, assessment and transition planning for high-risk patients in the ED setting.

Will float to cover inpatient medical surgical units on occasion

Focus is on connecting clients to the community setting

Weekend commitment currently 1 weekend in 5-week rotation

Qualifications

• 2+ years of appropriate clinical experience in utilization review or discharge planning experience preferred.

• 4+ years hospital care and discharge planning preferred.

• Graduate of an accredited school of Nursing or a BSW required.

• Master's degree in Nursing or Social Work preferred.

• Licensed as an LICSW in the state of Massachusetts required.
