POSITION TITLE Associate Case Manager

SUMMARY Help residents achieve and maintain optimum social, mental, spiritual and emotional well-being. Serve as a resource for residents and their families.

ESSENTIAL JOB 1. Meet with residents and families regularly. Work with family members to maintain constructive ties and / or ameliorate conflicts.

RESPONSIBILITIES 2. Complete a social assessment and develop and coordinate an Individualized Service Plan (ISP) on all residents.

3. Help residents make the adjustment to their new home.

4. Develop a formal and informal flow of communication and cooperation with all other staff members who serve the resident.

5. Be involved with and provide support to residents and families

when transfer to an alternative care setting is necessary.

6. Provide input and assistance to the activity program.

7. Participate in community activities involving the mentally ill.

8. Provide counseling services to residents in the event of crises in

their lives.

9. Identify and refer residents for counseling who are dealing or have a history with alcohol, substance abuse or depression.

10. Work with residents to identify their unmet needs.

11. Work in conjunction with the Administration to arrange for an aftercare program which is sufficient to that residents' needs.

12. Work with other resource people to coordinate resident services, located inside or outside the Facility. These may be provided by formal or informal helpers (i.e. , staff people, professionals or family members and volunteers).

13. Note and notify the appropriate authority if there is a change in the resident's mental or physical status.

14. Develop brief case records on each resident upon admission (initial), and then when incidences warrant, also annually, to provide continuity of care.

15. Arrange for service in areas of income maintenance, health, mental health, and social service. Assist residents as they apply for income and public benefit entitlements.

16. Develop a resource file on services, benefits and entitlements for residents.

17. Visit residents when they are in the hospital

18. Maintain updated records on residents' benefits (i.e. Social Security, SSI, HR, Pensions, Medicaid, Medicare etc.)

19. Ensure that Medical benefits are current and in effect.

20. Facilitate the remembering of birthdays, anniversaries, and illnesses and deaths with cards.

21. Assist with the resident council.

22. Assist the Administrator as a representative of the Facility in the community.

23. Review residents service needs periodically and acts as their spokesperson when necessary. Note complains and assist in their resolution.

24. Ensure that all residents' have an updated Medical and Mental Health Form filled out on an annual basis and when there is a change in the medical condition.

25. Carry out other responsibilities as assigned by supervisor

WORKING 1. Adhere to and carry out all policies and procedures.

BEHAVIORS 2. Report on-the-job injuries to the supervisor before the end of the

work shift on the day the injury occurs.

3. Establish and maintain constructive working relationships with

co-workers, residents, families and visitors.

4. Maintain confidentiality of verbal and written information

pertaining to residents, facility operations and personnel.

5. Promote The Sentinel of Rockland in a positive way.

6. Promote teamwork in providing services to resident.

QUALIFICATIONS

1. B.A degree from an accredited college or university with major work in human resource or service delivery.

2. One year experience, with the provision of services, to a depended adult population.

3. Flexibility and creativity in dealing with the issues being faced by

residents and families

4. Ability to exercise independent judgment within the

organization's mission and philosophy

5. Experience and enjoyment in working with older adults

STANDARDS

1. Residents always come first, and their needs will be met unless

there is a conflict with the needs of others or the organization as a

whole.

2. Employees will not work under the influence of mood-altering or

illegal drugs or alcohol.

3. Neat appearance, adherence to the dress code and good personal

hygiene are expected.

4. Must be available for all training and in-service sessions.

5. May be expected to perform tasks that are not included in this position description, but which may be assigned time permitting.
