Job Description

Purpose of Position:

To engage and connect patients with chronic health conditions to supportive programs, such as Lincoln's onsite diabetes self-management classes and our Tobacco Treatment Program.

As a clerical Associate in Ambulatory Care:

To systematically approach patients by registering and processing patients entering the practice and to accomplish the following:
  • To clearly delegate and outline individual clerical responsibilities and serve as a guide and reference.
  • To help contribute towards adequate and effective continuity of patient care by providing for sustained and efficient patient flow and productive utilization of clerical personnel.
  • To facilitate accurate and timely billing and reimbursement.
  • To enhance the patient experience and facilitate practice flow.
  • To augment and enhance work performance and morale of clerical personnel.
  • To ensure proper and timely revenue workflow.


Under the direction of the Chronic Disease Manager, the Program Assistant's duties Include:
  • Coordinating various chronic disease programs, including Lincoln's Tobacco Treatment Program and the diabetes self-management classes offered in partnership with the NYC Department of Health and Mental Hygiene;
  • With respect to such programs, duties include reaching patients, publicizing the programs by creating and posting flyers and other material, tracking enrollment numbers, and attending huddles and meetings;
  • Being present for the diabetes self-management classes which are held once a week in the evenings (usually a Wednesday);
  • Other tasks as needed.

Additional Duties:
• Answer telephone, relay messages, and directs accordingly.
  • Adhere to Outpatient Referral Procedures when indicated.
  • Is responsible for enhancing personal and professional development through attendance at required and mandated training sessions.
  • Is responsible for wearing professional attire to the workplace as well as during off-site work when conducting business on behalf of Lincoln Hospital Center, as per dress attire policy.
  • Promote a positive and pleasant attitude conducive to the workplace, which enhances and furthers the Mission, Vision, and Value statement.
  • Meet all compliance standards/requirements such as annual physical evaluation/assessment, Annual assessment Training; ELM training, and Fire Drill participation; annual OHS compliance
  • Adheres to the Mission/Vision/Value Statement and Guiding Principles of the Organization in all endeavors.

Supervisor additional duties:
  • Assists in the preparation of statistical data reports for patient quality improvement/regulatory monitoring activity.
  • Oversee practice operations as needed
  • Supervises performance of clerical staff and/or practice workflow
  • Sends "No Show" letters as needed.
  • Records intake time for cycle time processing.
  • Monitors and completes Work Queues


Minimum Qualifications
1. A high school diploma or its educational equivalent; and
2. Six (6) months of full-time satisfactory clerical experience and/or experience interacting with the public, such as processing sales; taking, processing, and delivering orders; obtaining and dispensing information; listening to and resolving customers concerns, problems, and complaints; and/or performing related customer service activities.
