  • JOB SUMMARY
    • The Clinic Coordinator- Rehab Services coordinates Physical Rehab Services provided in the inpatient and/or outpatient settings. Assists with management of peer review and Quality Improvement programs, provides mentorship and coordination for staff educational growth, assists with staff development and feedback, and assists manager with other duties (such as budgeting, equipment supplies and ordering, payroll assistance) and provides patient care consisting of the evaluation and treatment of patients, documentation of care and supervision.
  • MINIMUM QUALIFICATIONS
    • Education: See Licensure
    • Experience: Two (2) years experience as a clinician.
    • Licensure/Certification/Registration:

      WI Locations : Must be licensed to practice in PT, OT or ST in Wisconsin. Wisconsin Background Study upon hire and renewed per Wisconsin state law.

      MN Locations : Must be licensed to practice in PT, OT or ST in Minnesota.

      Must have Wisconsin License adn Wisconsin Background Study upon hire and renewed per Wisconsin state law if floating to WI Clinic Location.

  • KNOWLEDGE, SKILLS AND ABILITIES
    • Extensive knowledge in the field of Rehab, including patient assessment, treatment planning and documentation guidelines. Knowledge of and commitment to the team approach to rehabilitation; knowledge of code of ethics, safety practices and procedures.
    • READING - Intermediate: Ability to read and interpret documents such as operating and maintenance instructions and procedure manuals.
    • WRITING - Intermediate: Ability to write routine reports, correspondence, or procedures.
    • SPEAKING - Intermediate: Ability to effectively present information in one-on-one, small group situations or before groups of customers, clients, and other employees of the organization.
    • MATHEMATICAL SKILLS - Basic Skills: Ability to add and subtract two-digit numbers and to multiply and divide with 10's and 100's. Ability to perform these operations using units of American money and weight measurement, volume, and distance.
    • REASONING ABILITY - Intermediate Skills: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
    • AGE SPECIFIC COMPETENCIES - Skilled at assessment and knowledgeable of growth and development. Provides appropriate care respective to the ages of the patients served per clinical environment.

      • In an average day the employee is required to; sit for one (1) to two (2) hours; stand for three (3) to four (4) hours and walk for three (3) to four (4) hours. While performing the job, the employee is also occasionally required to; bend/stoop; squat, kneel, climb stairs, push/pull, balance, reach above shoulder level, reach below waist level, crawl and lift unwieldy stuff. Ability to lift up to fifty (50) pounds.

    • CRITICAL DEMANDS
      • The employee will be occasionally subject to varying weather conditions, as portions of the job require that the employee travel to other facilities. Also, portions of evaluation/treatment of patients may be conducted outdoors and include uneven terrain, climbing ladders, exposure to exhaust fumes and extreme temperatures. There is the potential for the employee to be in contact with hazardous cleaning agents and the associated fumes.


Additional Information
This position will have a focus on outpatient services support and growth. It is salaried. Will continue to be heavily patient care oriented with a portion of the FTE dedicated to non-patient care rehab department support duties.
