Clinic Counselor - Ann Silverman Community Health Clinic

Job Description:
Clinical counselor provides services in English and Spanish as well as risk assessment, case management, and appropriate referrals for patients, families and significant others served by the Ann Silverman Community Health Clinic (ASCHC). Assist patients and families in their adjustment to illness, treatment, disability and loss. To assess and plan for the patient's continuing care needs. To assist the Executive Director in implementing new program initiatives related to development of behavioral health capabilities.

Job Qualifications:

• QUALIFICA TIONS:

• Education: Bachelor of Science degree in Social Work or Human Development and Family Studies or related fields required. Licensed Professional Counselor (LCP) or Master of Social Work Degree preferred.

• Experience: Minimum of two years of recent experience in human services work such as case management, counseling or social work, preferably in an acute care hospital or other health care setting.

• Language: Fluent in both English and Spanish

• Other Skills: Highly refined assessment, organizational, verbal, written, interpersonal, prioritizing and problem solving skills.

• LSW or LCSW State of PA is preferred.
