  • Work environment: The My Home My Care team manages patients in Post-Acute Care (PAC) settings to include: Skilled Nursing Homes, Assisted Living Facilities, and the home environment. Our team works closely with PAC facilities, patients, families, and other patient care team members to meet the needs of our patient in their current environment. Our team is comprised of dedicated Advanced Practice Clinicians that provide care beyond the walls of the hospital, RNs who provide in depth care coordination for complex patients, and LPN's who provide office and triage support for the team.
  • Work hours: Full-time, Monday-Friday, no weekends
  • Salary range is based on relevant years of experience

Qualifications
  • Completed certificate or diploma program from a state approved nursing program or Medical Assistant certification.
  • Dual license preferred for WI and MI. Upon hire may be ask to obtain if employee doesn't already have license in both states.
In addition to the required qualifications, a successful LPN/MA will:
  • Demonstrate a high degree of confidentiality, positive interpersonal skills, and ability to function in a fast-paced environment.
  • Maintain a positive, customer-focused attitude toward patients
  • Possess a strong work ethic and always display a high level of professionalism
  • Have basic computer skills using the EPIC electronic medical record system
Position Responsibilities
Serves as a liaison among the patient/family, community services, PCP, specialists, and other care team members to coordinate services without duplication.

Provide continuous education to patient/family on health information, medications, and available community resources

Create comprehensive care plans and care management interventions to close care gaps and achieve patient's care goals. Update care plan as clinical progression occurs.

Delivers care to patients

· Performs diagnostic functions and data collection, and communicates variations from normal

· Assists providers and team with patient care needs.

· Transition of care focus to increase communication, remove transitional barriers, and assure patients are in the least restrictive environment.

· Active role in care coordination, communication, and collaboration to achieve and improve patient outcomes, compliance and satisfaction.

· Frequent monitoring of patient medical condition and diagnosis in collaboration with multiple disciplinary care team to develop/update goals of care.

· Health maintenance monitoring and compliance
    · Provide patient/ facility education to promote best outcomes
