Heartland Behavioral Health Services has been providing care for children and adolescents for over 100 years in Nevada, Missouri. We treat male and female patients ages 4 through 17 in both acute as well as residential programs for conditions ranging from depression, anxiety and other mood disorders to recovering from a traumatic event . The Outpatient Clinicial Care Coordinator provides support in the day-to-day operations and the coordination of care within the outpatient clinic. Serves as a liasion between the patient and the clinical team members. Ensures proper scheduling of appointments. Coordinates care with community primary care providers, schools, and other healthcare entities. Provides utilization management documents, pre-authorization for medications and coordinates new admission activities. Ensures that the outpatient medical charts comply and meetings all standards of care. Supports various insurance and financial

counseling functions related to ensuring financial payments and follow-up paperwork completion. Performs back-up clerical functions throughout the facility in a supportive role.

Job Duties/Responsibilities:
  • Supports intake and utilization management areas by assisting in daily tasks such as admitting new patients, gathering, and organizing the necessary documents needed for continued authorization
  • Supports outpatient clinic process by assisting with processing new patient paperwork. Ensures that the process flows smoothly and gathers needed documents from community providers
  • Supports continual updating of clinic sessions used and utilization management information into the computer accurately and in a timely manner.
  • Schedules appointments, contacts patients prior to appointments to reduce no-shows. Screens in new patients as they enter the outpatient center.
  • Completes verification of data entry for patient demographics, authorization, and payer/plan eligibility within the patient record prior to each visit.
  • Coordinated outpatient physician and therapist schedules.
  • Completes the initial patient admission process for patients new to the HBH system.
  • Demonstrates proficiency in telehealth scheduling and functioning, in MS4 use, Concord and Midas. Answers the phone/fax system on clinic operating days.
  • May provide support role to provide breaks, one day a week to the switchboard, serves as a back up to utilization care management in the hospital and RTC levels of care. May aid with chart filing in various departments within the facility.
  • Assists with communication with families and updates as to the status of treatment, coordinates the transition of care from acute and RTC to outpatient level of care.

Qualifications

Requirements:
  • Graduate of high school diploma or equivalent
  • At least 2 years' experience working in the mental health field
  • Specific experience in the care management and/or utilization review.
  • Individual will have strong working knowledge of insurance verification, pre-authorization for medication, clinical chart record requirements
  • Individual will demonstrate an ability to utilize Microsoft Word, Excel, telehealth scheduling and other hospital specific programs
  • Individual will be detail orientated and have an experience in providing quality customer service.
  • Individual will have experience operating in the role of a clinic manager or business office manager.
