Responsibilities

REPORTS TO : Director of Clinical Services

POSITION PURPOSE : Provides support and assists with clinical case management to include: researching community referrals, long term treatment facilities, providing case management support/services to the clinical team (making phone calls, faxing collateral information, scheduling clinical appointments, etc.)

KEY RESPONSIBILITIES :

  • Maintain an up to date list with contact information for: outpatient referrals, residential treatment centers, nursing homes, assisted living and sober homes, including which has current openings.
  • Establishes and maintains good relationships with key contacts.
  • Obtains ROI from patients and faxes or emails collateral information to potential outpatient providers and treatment facilities.
  • Works in partnership with the therapist and Physician to facilitate compassionate patient communication and advocacy, effective Treatment Team planning and timely discharge planning.
  • Assists the clinical team with case management to include: faxing collateral, filing notes, scheduling sessions, family/support system contact, and other duties to be assigned.

  • Qualifications

    QUALIFICATIONS :

    SKILLS, KNOWLEDGE AND ABILITIES: Good organizational, verbal and written skills. Ability to maintain confidentiality. Ability to form and maintain professional relationships with community partners. Some knowledge of Joint Commission, CMS, Tricare standards.

    EDUCATION: Bachelor's degree in a related field preferred or High School Diploma with 2 or more years of experience in mental health field.

    LICENSURE/CERTIFICATION: Not required

    EXPERIENCE: At least two years' experience in mental health setting preferred.

    PHYSICAL REQUIREMENTS:

    * Correctable vision and hearing.

    * Must be capable of occasionally lifting 25 pounds and carrying medical records.

    * Must be able to write and record information, and accurately read medical records.

    * Must be able to spend 30% to 40% of time standing or walking between units. 60% to 70% of the time is spent sitting at a desk.

    * Telephone or public face-to-face contact is infrequent, but must be able to calmly handle frustrated or angry clients.
