The role you'll contribute:
The Collaborative Clinical Coordinator is a key and pivotable role in prevention of readmissions for our AdventHealth Central Florida Division. The Clinical Coordinator will work out of our Mission Control department and will act as a centralized system resource. This position requires excellent critical thinking skills and enhanced communication skills with all members of the multi-collaborative team. The Clinical Coordinator will focus on readmission prevention and emergency department discharge excellence. They will advise and execute on resources and strategies available to campuses to prevent readmissions, including post-acute bridging. Additionally, the clinical coordinator will review readmissions real-time that can come into any emergency department and look at weekly data analysis for trends and opportunities. They will have the opportunity to guide and create improvement interventions around identified trends. There is an exciting opportunity to engage with patients who have recently been discharged and monitor and escalate any needs from the 24-7 hotline.

- Under direction of director and manager, collects, synthesizes and shares data trends with the interdisciplinary care team, driving process improvement strategies.
- Participates in deployment of acute nursing support strategies
- Coordinates campus metrics and demographics for benchmarking submissions.
- Coordinates annual review process of policy, SOP and education documents for nursing to ensure compliance with CMS regulations.
Qualifications
The expertise and experiences you'll need to succeed:
Minimum qualifications:
- Associate of Science in Nursing
- Current valid State of Florida or multi state license as a Registered Nurse
Preferred qualifications:
- Bachelor's degree in Nursing
- Previous Case Management experience
- Previous Emergency Department experience
