The Nurse Care Manager (NCM) is responsible for working with and providing clinical support for an identified group of patients and families, managing their ongoing health treatment plan, with the goal of optimizing health. The NCM actively supports the complex clinical care needs of the patients cared for by the provider they are aligned with in the outpatient setting.
