Vantage at Chelmsford is seeking a self-motivated individual to join their team. The ideal candidate should ensure effective and successful relationships are maintained with the referral sources resulting in skilled admissions that meet the census goals of the center.

REMOTE POSITION!

Responsibilities

• Manage all aspects of the referral and admission process. Receive and evaluate all referrals to assure clinical appropriateness for the hospital. Work with the hospital to ensure all steps of the admission process are thoroughly completed. Present a completed admission record to the facility.

• Maintain relationship with hospital Case Manager to ensure all steps of the admission process are thoroughly completed.

• Communicates with the families of patients, offering bedside sales along with tours of the facility.

• Maintain excellent working relationships with medical professionals and other health related facilities and organizations.

• Coordinates with healthcare facility staff to ensure that each patient has a smooth transition through the discharge process.

• Attend and participate in facility meetings, calls and in-services

• Works alongside your Director of Business Development to bring awareness of the facility's strengths and services into the hospitals, case managers, specialty clinics and physicians.

• Ensure daily reports are accurate for appropriate follow-up the next day.

• Build a strong network relationship, engage in person-to-person calls and meetings with the referral sources to exposure for company portfolio.

Knowledge/Skills/Ability:

• Ability to project a professional image.

• Knowledge of regulatory standards and compliance requirements.

• Working knowledge and ability to apply professional standards of practice in job situations.

• Communication skills- both written and oral- must be exemplary.

• Must possess strong clinical assessments and process skills.

• Must have excellent computer skills and knowledge of databases.

• Knowledge of medical terminology.

• Ability to make independent decisions when circumstances warrant.

• Strong interpersonal skills.

• Must be customer service oriented.

• Must have excellent organizational skills and be an effective time manager and multi-tasker.

License/Certification:

• Must have a valid driver's license

• RN/LPN/LSW preferred but not required

Experience:

• Two years clinical and marketing experience in a multiple service healthcare model with an emphasis on network and business development.

• Working knowledge of the various electronic referral platforms.
