MAIN FUNCTION:
Under the direction of the Director of Pharmacy, collaborates with interdisciplinary healthcare teams to provide specialized therapeutic knowledge, experience, and judgment to provide optimal patient outcomes. Reports to the Director of Pharmacy.
DUTIES AND RESPONSIBILITIES:
** 1. Participates in interdisciplinary inpatient rounds, collaborates with other healthcare professionals, and represents pharmacy in a knowledgeable and professional manner; the clinical rounding pharmacist will collaborate to provide the most optimal, cost-effective utilization of medications.
**2. Participates in the development of policies, procedures, and guidelines for clinical pharmacy activities throughout Arnot Health.
**3. Coordinates Medication Use evaluation and quality initiatives.
**4. Communicate with all staff members and provide information relevant to clinical activities.
5. Participates in clinical meetings and relevant committees including Pharmacy and Therapeutics, Falls, or or others.
6. Maintains Smart Pump libraries.
7. Remains abreast of developments in the profession of pharmacy through continuing education, review. of professional literature, and communication with peers and professional organizations.
**8. Documents pharmaceutical care in the patient's medical record as appropriate. Maintains appropriate documentation related to the cost impact of the clinical activity quality improvement process.
9. Coordinates and performs drug therapy assessment, monitoring, and documentation while communicating relevant findings and recommendations to other healthcare professionals.
10. Coordinates and provides patient education and counseling regarding drug therapy and drug-related disease prevention as required.
11. Assists the department in addressing physician consults as needed. Provides timely retrieval of reliable drug information from available sources.
12. Provides education of health care practitioners regarding drug use as appropriate.
13. Aids and collaborates with the Antimicrobial Stewardship program, as required.
14. Assumes preceptor responsibilities for specific Pharmacy student externs during their clinical rotation from affiliated Colleges of Pharmacy as registered preceptor.
15. Performs all those activities and all the job functions and responsibilities of a staff Pharmacist as required.
24. EDUCATION:
Graduation from an ACPE accredited school of pharmacy with an advanced degree of PharmD is required. Pharmacy New York State License or License eligible required.
EXPERIENCE:
Three years of experience in a comparable clinical setting is preferred.
CARDIOPULMONARY RESUSCITATION (CPR) REQUIREMENTS:
Basic Life Support (BLS) required within 90 days of hire date.
PHYSICAL DEMANDS:
Some physical effort required. Light objects lifted, some stooping and reaching. Lifting of 15lbs frequently, lifting of 40lbs occasionally.
EXPOSURE CATEGORY:
Category III, tasks that involve no exposure to blood, bodily fluids or tissue. And Category I tasks are not a condition of employment.

- 16. The Clinical Pharmacist must demonstrate growth and development of knowledge required to assess data reflective of the patient status. Interpret the appropriate information needed to identify each patient's requirements and to provide the pharmaceutical care needed appropriate to the age of the patient and in accordance with appropriate polices and procedures. Reflective of these requirements, the Clinical Pharmacist must:
- Demonstrates competency in monitoring renal function and making dosage adjustments based on creatinine clearance.
- Demonstrates competency in monitoring drug regimens and making recommendations in the following areas:
- Antibiotic utilization; therapeutic appropriateness and duplication; assessment for potential interactions i.e. drug-drug, drug-food, drug-laboratory, drug-disease; Assessment of clinical and pharmacokinetic laboratory data to evaluate drug efficacy and anticipate drug toxicity or adverse effects.
- Provision of accurate and comprehensive information about drug and patient-specific drug information to other pharmacists, health care professionals and patients as appropriate.
- Is adept in documentation of clinical activities.
- Actively participates in one of the professional pharmacy organizations.
- Participates in relevant ACPE continuing education programs and maintains documented proof of CE credits earned as required by the NYS Education Department.
- Demonstrates age specific competencies to meet pharmaceutical care needs of neonatal, pediatric, adolescent, adult and geriatric patients.
- Is responsible for attending all annual mandatory educational programs as required by position.
